Remember:

• Wear a shirt that gives easy access to your high upper arm.

• All those age 17 and younger must have parent or legal guardian present. 
• Vaccines given to those age 9 years and older. 
• On an individual basis the Registered Nurse may approve vaccination for those 7-8 years of age.

• Employees with current coverage with Hometown Health will be required to present their Insurance Card AND Driver’s License at the time of vaccination.

• No Out-of Pocket Costs: 
• Hometown Health – Fully Insured HMO/PPO plans
• Senior Care Plus   
• State of Nevada – Hometown Health(HMO plan) & PEBP(PPO plan)
• Medicare – Primary, with no other HMO (Must have part B)
• WCSD – HMO/PPO

• For all others the cost is as follows:
• $35 Flu Shot
• $60 High Dose
• $105 Pneumovax 23
• $205 Prevnar 13
